Tuesday Keynote Presentation: From Evidence to Impact: Changing the Menu with Food as Medicine to Treat and Reverse Chronic Disease

Join us to explore how food as medicine is reshaping chronic disease care. Innovative leaders from diverse backgrounds, including academic research, clinical and community-based models and integrated payor/provider systems, will discuss evidence-based research demonstrating the clinical impact of nutrition interventions in preventing, treating, and reversing disease. Scalable food as medicine interventions in healthcare systems and communities across the country will be highlighted, including financial mechanisms—like value-based payment models and public insurance innovations—that are fueling sustainable program growth. This session will explore strategies for integrating food as medicine into clinical workflows, team-based care, and community partnerships; as well as emerging research, policy shifts, and implementation trends that will define the next chapter in nutrition-informed healthcare.

Learning Objectives

  1. Analyze clinical and implementation research on food as medicine interventions for the prevention, treatment, and reversal of chronic disease.
  2. Describe successful models of food as medicine programs currently in practice across the U.S., including clinical and community-based approaches that demonstrate scalability and sustainability.
  3. Evaluate financial models that support the implementation and expansion of food as medicine initiatives, including integration into value-based care and government-sponsored insurance programs.
  4. Explore strategies for operational integration of food as medicine into clinical workflows, including interdisciplinary team roles and partnerships with community food systems.
  5. Discuss future developments in food as medicine policy, research, and clinical application to inform ongoing practice transformation and advance chronic disease care.
