BLUE ZONES CERTIFIED™ PHYSICIAN AND BLUE ZONES CERTIFIED™ HEALTH PROFESSIONAL TERMS AND CONDITIONS
Terms and Conditions
The Blue Zones Certification Course for Physicians and Health Professionals (the “Course”) is offered jointly by Blue Zones, LLC (“Blue Zones”) and the American College of Lifestyle Medicine (“ACLM”).
You, on behalf of yourself (“You” or “Your”), hereby agree that the following Terms and Conditions (“Terms”) shall govern Your post-completion activity with regard to the Course and use of the Blue Zones Certified designation, and that Blue Zones and ACLM may update these Terms at any time.
- General Rules
a. You acknowledge that Blue Zones and ACLM may change or modify any rules, regulations and directions associated with the Blue Zones Certified designation at any time, and that Blue Zones and ACLM have the right to retain all information submitted by You during and after the Course.
b. You hereby acknowledge and agree that all information and materials provided by Blue Zones and/or ACLM during the Course (“Course Materials”) constitute confidential information and are owned by Blue Zones and/or ACLM. The Course Materials are provided solely for Your use (i) in connection with Your participation in the Course, and (ii) pursuant to the terms set forth herein. Except as expressly set forth herein, You may not reproduce, prepare derivative works from, distribute, display, or perform the Course Materials, use them in workshops or other training sessions, or use them for any other purpose without Blue Zones and ACLM’s prior written consent.
c. You hereby acknowledge that any use of the Blue Zones Certified designation and Course Materials is contingent upon Your compliance with these Terms, and failure to comply will subject you to probation and/or revocation of Your Blue Zones Certified designation and use of the Course Materials.
d. You hereby acknowledge that Blue Zone and ACLM reserve the right, in their sole discretion, to revoke Your Blue Zones Certified designation and use of the Course Materials at any time for any reason, including but not limited to if You engage with or perform services for a business or entity that would pose a conflict with Blue Zones or ACLM.
e. You agree that if Your Blue Zones Certified designation has been revoked or otherwise terminated by Blue Zones, You shall immediately destroy, delete and dispose of any Course Materials cease all use of the Blue Zones Certified designation.
f. You hereby acknowledge that you have received a copy of Blue Zones' Privacy Policy and ACLM’s Privacy Policy which shall apply to the collection and processing of all personal data included or provided in connection with these Terms and Conditions.
- The Blue Zones Certified Designation. The Blue Zones Certified designation signifies specialized knowledge and competency in the history, research, and modern-day application of Blue Zones principles, and dedication as a lifestyle medicine certified physician or health professional to integrate lifestyle medicine and community-based strategies to promote well-being and improve health outcomes in both clinical and community settings. It can be earned by a lifestyle medicine certified physician or health professional through the completion of all Course modules, earning a successful passing score on all related assessments, signing a required attestation that they will complete the workbook to the best of their ability, and automated validation of Lifestyle Medicine Certification through the American Board of Lifestyle Medicine, the American College of Lifestyle Medicine, or the International Board of Lifestyle Medicine.
Maintaining the Blue Zones Certified designation requires: (i) maintaining Lifestyle Medicine Certification and eligibility status; and (ii) remaining in good standing and in full compliance with all applicable professional, ethical, and legal standards by applicable licensing authorities. Blue Zones and ACLM reserve the right to deny, suspend, or revoke your Blue Zones Certification at any time for failure to meet these requirements.
- Permissible Use of Blue Zones Certified Designation. The following are permissible for using the Blue Zones Certified designation upon completion of the Course and related requirements:
● Email and Professional Signature: You may use the official digital badge or digital signature provided by Blue Zones/ACLM (the “Badge” and the “Signature”), or include the credential of “Blue Zones Certified Physician” or “Blue Zones Certified Health Professional” as part of your email signature or professional letterhead signature.
● CV or LinkedIn: You may include that you have completed the Course, earned the certification, and/or include the applicable “Blue Zones Certified Physician” or “Blue Zones Certified Health Professional” designation on your CV, LinkedIn profile, or other professional bio.
● Websites and Social Media: You may include the Badge or Signature on your personal or professional website(s) and/or social media accounts.
● Displaying the Course Certificate of Achievement: You may hang-up your Certificate of Achievement for the Certification in your personal or professional practice/office space, and share on social media.
● Other: You may reference Your participation in and completion of the certification Course in a non-commercial, informational, or educational manner, so long as you refrain from suggesting, implying or stating that your professional practice is officially sponsored, endorsed, or affiliated with Blue Zones or ACLM.
- Prohibitions on Use of the Blue Zones Certified Designation.
● Other than displaying the Course Certificate and use of the Blue Zones Certified designation as permitted above, you are not permitted to otherwise use or display any of Blue Zones or ACLM trademarks or logos in connection with Your personal or business activities.
● The Blue Zones Certified designation is an individual designation. Please refrain from any conduct that would imply that your clinic or practice is Blue Zones certified.
● While you may state you completed the Course and are Blue Zones Certified, you are not a Blue Zones representative or advocate.
● Overall, please refrain from any conduct that would suggest that You, Your professional practice/business, or Your products and/or services are officially sponsored by Blue Zones.
- Limited Permission to Use Course Materials. Your completion of the Course and Blue Zones Certified designation confers upon you limited rights to use the Course Materials in accordance with the following:
● For Course Materials designated by ACLM and Blue Zones as “Tools,” which may include checklists or worksheets, You may print and distribute such Course Materials for (i) non-commercial, personal use, and (ii) for use by clients and patients in accordance with these Terms and Conditions. In no event shall you distribute any Course Materials to any other person or charge for such Course Materials.
